NTSB final report: Bell UH-1B, Amherstdale, West Virginia, Jun. 22, 2022

The National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be the operator’s failure to adequately inspect the former military helicopter, which allowed an engine issue to progress, resulting in a loss of power. The helicopter struck power lines during a forced landing. Investigators also cited three contributing factors: inadequate FAA inspection and maintenance standards for former military turbine-powered aircraft operating with an experimental exhibition airworthiness certificate; the operator’s use of the FAA standards instead of more rigorous standards; and the FAA’s inadequate oversight of the operator. Six people died in the accident.

FAA proposes special conditions for Skyryse-modified Robinson R66

The FAA is seeking public comments through Oct. 15, 2024, on the special conditions, which contain additional standards to establish a level of safety equivalent to existing airworthiness standards. The control input and fly-by-wire system on the Skyryse variant of the Robinson R66 are not covered under current FAA standards.

Proposed FAA AD: MD 369 series, 500N, and 600N helicopters

The FAA is accepting comments through Oct. 15, 2024, on this proposed airworthiness directive (PAD) that would require repetitively inspecting (and replacing, if necessary) certain part-numbered tail-rotor pedal support brackets. It would also prohibit installing certain tail-rotor pedal support brackets.
