NTSB final report: Bell 407 and Sikorsky S-64E, Cabazon, California, Aug. 6, 2023

The US National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be the S-64 flight crew’s decision to enter a fire traffic area at an altitude significantly above their maximum cleared altitude, which resulted in the need for an aggressive descent into congested airspace and a failure to see and avoid the Bell 407.
