The helicopter struck power lines during a forced landing. Investigators also cited three contributing factors: inadequate FAA inspection and maintenance standards for former military turbine-powered aircraft operating with an experimental exhibition airworthiness certificate; the operator’s use of the FAA standards instead of more rigorous standards; and the FAA’s inadequate oversight of the operator. Six people died in the accident.
