ATSB final report: Robinson R44, Pilbara, Western Australia, Nov. 3, 2022

The Australian Transport Safety Bureau (ATSB) has identified the pilot’s failure to review or recalculate the performance of the helicopter for density altitude and weight and taking off from a confined area as factors that led to the accident. The ATSB also noted that the passenger-carrying flight should have been operated under Part 133, as the operator was only authorized for aerial work operations.
