The accident resulted in the deaths of a student pilot and an instructor. VAI Daily Aug. 28, 2024 NTSB final report: Bell UH-1B, Amherstdale, West Virginia, Jun. 22, 2022 The National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be the operator’s failure to adequately inspect the former military helicopter, which allowed an engine issue to progress, resulting in a loss of power. The helicopter struck power lines during a forced landing. Investigators also cited three contributing factors: inadequate FAA inspection and maintenance standards for former military turbine-powered aircraft operating with an experimental exhibition airworthiness certificate; the operator’s use of the FAA standards instead of more rigorous standards; and the FAA’s inadequate oversight of the operator. Six people died in the accident.
