A US Coast Guard (USCG) Sikorsky MH-60 Jayhawk crew from Clearwater, Florida, hoisted two boaters Aug. 4 off the coast of Boca Grande after they lost their sail during Tropical Storm Debby.

ATSB final report: Robinson R22, Gold Coast Airport, Feb. 26, 2022

The Australian Transport Safety Bureau (ATSB) is reminding Robinson R22 pilots and maintainers about the importance of visual inspections for corrosion. Investigators determined that the failure of a tail-rotor tip cap adhesive caused the separation of the tail assembly that led to the accident during a low-level training exercise at Gold Coast Airport (YBCG) near Brisbane, Queensland.

NTSB preliminary report: Schweizer 269C, West Carthage, New York, Jul. 27, 2024

The National Transportation Safety Board (NTSB) has released a preliminary report that indicates a witness heard an engine “sputter” before the helicopter fell. Investigators found evidence of a wasp’s nest in the vent tube leading to the fuel tank. The pilot died in the accident.
