(NPR) – A US Coast Guard helicopter crew rescued a boater in distress and his dog Sep. 26 in waters off Sanibel Island, Florida, ahead of Hurricane Helene making landfall.

The National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be the disbonding of a tail-rotor driveshaft splined adapter, which resulted in a loss of tail-rotor drive and a hard landing.
