VAI Daily May. 7, 2025 NTSB final report: Airbus Helicopters EC130, Halloran Springs, California, Feb. 9, 2024 The National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be the pilot’s decision to continue a VFR flight into instrument meteorological conditions, which led to spatial disorientation and a loss of control. Investigators identified the operator’s inadequate oversight of its safety management processes as a contributing factor in the accident, including ensuring the pilots were accurately completing and updating the flight risk analysis, logging maintenance discrepancies, and ensuring the helicopter met Part 135 regulations before departure.
