RCAF final report: CH-147F, near Petawawa, Ontario, Jun. 20, 2023

The Royal Canadian Air Force (RCAF) Directorate of Flight Safety has concluded that multiple factors contributed to the spatial disorientation of all four crew members during a night training flight over the Ottawa River, which led to a controlled-flight-into-terrain accident. Two pilots died in the accident.
