NTSB final report: Bell 206, Greenville, Illinois, Nov. 2, 2022

The National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be the maintenance technician’s failure to inspect and clean the entire fuel-nozzle assembly in accordance with the engine maintenance manual, which resulted in fuel starvation and a partial loss of engine power.
