NTSB final report: Robinson R44, Honolulu, Hawaii, Aug. 18, 2024

The US National Transportation Safety Board (NTSB) has determined the probable cause of the accident to be a failure of the flexplate yoke flange assembly due to incorrectly torqued bolts. Investigators identified the maintenance crew’s inadequate inspection of the tail-rotor flexplate assembly as a contributing factor.

Opinion: Fixing what caused the midair collision near KDCA

(AIN) – Tim Lilley is a retired US Army Black Hawk pilot and father of one of the pilots who died in the Jan. 29 midair collision of a commercial jet and a US Army helicopter near Ronald Reagan Washington National Airport (KDCA).
